This Consumer Health Privacy Notice (\u201cNotice\u201d) is intended to supplement the Privacy Notice<\/a><\/span> and other applicable privacy notices (\u201cprivacy statements\u201d) to provide additional notices related to health information or health data (\u201chealth information\u201d) as defined by Washington and Nevada law. Washington and Nevada require us to provide you with additional information about how we may collect, use, disclose or otherwise process your health information, how we protect it, and your rights and choices with respect to your health information.\u00a0<\/span><\/p>\n

This Notice applies to all operations as well as websites, mobile applications and digital services (“Services”) that link to or post it.\u00a0<\/span><\/p>\n

Scope\u00a0<\/b><\/p>\n

Health information for purposes of this Notice has the definition given in Washington or Nevada law, including information that identifies or is reasonably capable of being associated or linked, directly or indirectly, with a particular consumer and that identifies the consumer\u2019s past, present, or future physical or mental health status. This Notice applies only to residents of Washington and Nevada and individuals whose information is processed in Washington or Nevada.\u00a0<\/span><\/p>\n

This Notice does not apply to the personal information that we collect and process about our workforce members, including job applicants; active and inactive employees; former employees; retirees; dependents; beneficiaries; employees of our third-party vendors and partners, such as contractors on assignment; and other individuals about whom we collect PI for HR-related purposes. It does not apply to health information that is used to engage in public or peer-reviewed scientific, historical, or statistical research that adheres to all other applicable ethics and privacy laws.\u00a0<\/span><\/p>\n

In certain circumstances, we may collect or use your information through our Services while acting as a \u201cbusiness associate\u201d under the Health Insurance Portability and Accountability Act of 1996 (\u201cHIPAA\u201d), and your information may constitute \u201cProtected Health Information.\u201d This Notice does not apply to Protected Health Information or any health information processed in circumstances where we are acting as a business associate under HIPAA.\u00a0<\/span><\/p>\n
